The above application forms are fillable-PDF documents. You can complete the form within Adobe Acrobat.

However, you will not be able to save the data that you have entered unless:
  1. You have Adobe Acrobat Professional; in which case you can save the PDF along with what you have typed in.
  2. You save from the PRINT menu under "SAVE AS PDF" (may not be available on your machine); this will "print" the form to PDF; note that you cannot modify the form once you've printed it.
Therefore once you've completed the form, be sure to save or print it; otherwise you may lose the data once you close the document.
